on your cover sheet.

days. If your request is urgent, you may call 303-860-9915
to ask for preapproval. Urgent requests are considered within
48 hours.
Criteria for Direct Assistance Requests
- Most requests supported by the Fund are
for items and services costing less than $250. Larger
amounts may be considered but must be approved by the
full Board of Directors.
- Applications must be submitted by a health
care, human service, or pastoral professional.
- Requests must be for specific items or
services that will directly enhance quality of life or
comfort for the patient.
- The recipient must be a person with end-stage
illness or his or her immediate, nonprofessional caregiver.
- The recipient must reside in one of these
Metro Denver area counties: Adams, Arapahoe, Boulder,
Broomfield, Denver, Douglas, Elbert, or Jefferson.
- Consent to request assistance and disclose
relevant medical and personal information must be obtained
from the patient or authorized agent or guardian before
you apply.
- Other resources (including the patient’s)
must have been explored and found to be insufficient or
unavailable.
- Applicants must notify Namaste Comfort
Fund of any significant change in the patient’s
circumstances related to the request while the application
is in process.
Namaste Comfort Fund does not discriminate on the basis
of race, sex, ethnicity, religious affiliation, or sexual
orientation.
Exclusions and Conditions
- Approval of requests is at the discretion
of the Namaste Comfort Fund Board of Directors and executive
director. Criteria, conditions, and exclusions are subject
to change and update without notice.
- In most circumstances, items or services
must be purchased after application approval.
- Assistance for items or services covered
as a benefit of another service that the patient is already
receiving (e.g., hospice, Medicare/Medicaid, home health,
long-term care, etc.) will not be approved.
- The patient-recipient must still be living
at the time the request for assistance is submitted and
at the time the item or service is obtained, unless the
assistance will directly ease a caregiver burden still
present at, or made worse by, the death of the patient.
- Typically, services provided through Namaste
Comfort Fund will be terminated on the patient’s
death. (Special circumstances may be considered by the
Namaste Comfort Fund executive director and Board of Directors.)
How Direct Assistance Funds Are Paid
Generally, items or services are paid for directly by
the Fund. In some cases the Fund will reimburse the
recipient or caregiver for an expense but only if the
expense (as evidenced by the submitted receipt) is for
under $100 and was incurred after the request
was approved.
